Failure to manage fatigue led to road fatalities

Railway welding specialist Renown Consultants has been fined £450,000 and ordered to pay an additional £300,000 in costs for failing to ensure that two of its workers – who were fatally injured in a road collision after a night shift – were sufficiently rested to work and travel safely.
